After our client died from pulmonary embolism, we filed a case against a hospital and our client's family doctor. During the case we discovered that our client had been seen by his family doctor and an emergency room doctor with symptoms of pulmonary embolism such as shortness of breath, fast heart rate (tachycardia), chest pain, and had been sedentary, yet his doctors did not order the tests needed to diagnose his pulmonary embolism. We argued that they should have ordered a VQ (ventilation-perfusion) scan. Instead, they diagnosed him with angina. This was a misdiagnosis.
